MTD Guidelines, Expectations & Benchmarks
All staff are required to fill out the time card form once a week prior to your scheduled management time. Hours are classified as "Ministry Team," "Team Player," "MTD," & "PTO/Holiday." Descriptions and explanations can be found on the form. Please see Lauren Ballard with questions.
If you are requesting Paid Time Off, reporting sick days, or indicating your absence because of work related travel or requirements, please use the form below. Your supervisor will contact you regarding the approval of your requested Paid Time Off.
Please fill out this form for Abby if you need her to write a check, put money somewhere else, add you to a budget, request more funds for a budget, or anything else that is finance related. Thanks!
